About the
Survey
Anyone can take the
survey. Parents, grandparents, couples without children and singles. You must register for the survey so
that we can update you and provide you with reports.
As our gift to you, all who complete their survey will receive the immunity building Healthy 4 Life, a #1-rated, colorful nutritional education handbook (50 pages, 50 recipes) by internationally known Sally Fallon-Morell.
The purpose of this comprehensive survey is to
measure any differences of immediate and long-term health outcomes (including
autism, ADHD, allergies, asthma . . .) of children and parents who are not
vaccinated, partially vaccinated and fully vaccinated within the same family
and for all participants combined. It will also ask about infertility,
miscarriage, prematurity, vaccinations given during pregnancy and the number
of vaccines given at the same time. The survey will also measure any
differences in health outcomes across two and three generations.
My Family Vaccination Survey
is sponsored by the Journal of Natural Food and Health and is supported
by parents, doctors, nurses, health professionals and organizations. The project
is funded privately and by gifts from participants. It is an independent,
not-for-profit project.

survey can be completed in more than one session– as long as “cookies”
have not been disabled on your computer.

Everyone completing the survey will receive the initial report with results in February 2013 and a final report in July.
What Good is an Internet Survey?

To our knowledge, no survey of this type has
ever been conducted in the U.S. The federal or state health authorities have
never conducted a study on the long-term health outcomes of vaccinated versus
non-vaccinated children and adults. Surveying is a scientific tool and can
provide valuable information and can point to areas for further study. The value
of the results of a survey is dependent on the kind of questions asked, who
takes the survey, how the data is interpreted and understanding the limits of
the data. Internet surveys are conducted continuously by state and federal
health agencies such as the CDC, your state health department, universities and their partners–even on
vaccinations–to determine policies and program funding.To minimize bias, we desire participants
that are pro-vaccination as well as those that question vaccine
safety and those that are largely opposed to vaccinations.
Privacy
& Confidentiality: We understand your vaccination and health information
is strictly confidential. We will never give your email out nor will it ever be
associated with the data you submit. Only the project director has access to
your email address for updating you and sending the survey.

MICHAEL, BSc, MSc, PEng
David's career spans forty years as
a licensed environmental, health and safety professional. He has developed
environmental, health and safety programs for Toyota, GM, Ryder, Firestone,
Marathon Oil Co., military and government agencies and provided guidance to
hundreds of others. He was formerly Chairman of the Environmental Resource
Institute. With 25 years in dealing with a broad spectrum of autism issues, he is the father of an autistic son and an advocate for vaccination choice and informed consent. David recently resigned his position at a
local health department after serving five years. His main interests are in health education and communications in food, water and vaccine safety. Now, he consults and speaks on a variety of topics and is the
publisher of the Journal of Natural Food and Health. He and his wife live in NE Ohio.
